The fertility narrative around age 40 is dominated by decline statistics and urgency. Women are told to move fast, consider donor eggs, and not get their hopes up. I want to offer a more complete and more honest answer especially as a woman who conceived naturally in her 40’s.
What the Research Actually Shows
Natural pregnancy rates decline with age. This is real. But they do not drop to zero. Plenty of women conceive at 40, 42, even 45 or 46, and I have worked with clients who did it at 45, 46, 47, and 48. The research consistently shows that egg quality, not just egg quantity, determines outcomes, and egg quality is modifiable.
A large IVF study analyzing over 20,000 eggs found that chromosomal abnormality rates increased with age, but not nearly as sharply as most women are told. In women 40 to 44, about 45 to 60 percent of eggs were still chromosomally normal. The researchers also noted that the aggressive hormonal stimulation used in IVF may be inflating those abnormality rates, which means the real numbers could be better. Eggs from women over 40 are not uniformly abnormal. Many are capable of supporting a healthy pregnancy.
What Gives You the Best Chance
Women who enter their 40s with low systemic inflammation, strong metabolic health, good nutrient status, and a well-regulated nervous system have meaningfully better fertility outcomes than population averages suggest. These are all modifiable.
The egg quality protocol remains essential, and it works best when you commit to at least three months, since the eggs you ovulate began maturing roughly 100 days earlier: CoQ10, omega-3 fatty acids, vitamin D, methylfolate, anti-inflammatory nutrition, sleep, and toxin reduction.

Accurate ovulation tracking matters more over 40, not less. Every cycle carries more weight, and missing your window is a higher cost. Sperm needs to be on its journey before the egg is released, so I recommend starting about four days before ovulation and having sex every 24 to 36 hours. Be cautious with basic ovulation kits, since many can miss your LH surge. Kits that also test progesterone confirm that ovulation actually happened.
When to Get Additional Support
If you are 40 or older, do not wait the full six months that applies to women over 35. If you have been trying without conception, or have any known concerns, a full fertility workup is warranted early. This is not a failure threshold. It is an information-gathering step. Understand your ovarian reserve alongside your overall health picture, and ask your partner to have a semen analysis along with a DNA fragmentation test, since a standard analysis can miss DNA damage. Natural conception, IUI, and IVF are not mutually exclusive. They can be pursued in sequence or in combination.

The oldest client I have helped conceive with her own eggs was 48 years old, and her day 3 FSH in the cycle she conceived her healthy baby girl was 29. Natural conception after 40 is not guaranteed, but it is possible, and the work you do to prepare for it is never wasted.
